About the Opportunity
Are you a motivated and forward-thinking leader, eager to make a difference in the drug and alcohol field?

The primary purpose of this position is to lead multi-disciplinary teams that provide intensive care coordination to support consumers with chronic and severe substance use issues, and mental health co-morbidities.

The ACM program and the DHS D&A/Mental Health Care Navigation Team target those with chronic and severe drug and alcohol dependence who experience multiple and complex needs. These teams will provide support to assist in the client's stabilisation and reintegration, and where relevant, assist in managing the client's underlying cognitive impairment.


What You'll be Doing
The primary purpose of this position is to lead multi-disciplinary teams that provide intensive care coordination to support consumers with chronic and severe substance use issues, and mental health co-morbidities.

This position will provide leadership to the ACM program and the DHS D&A/Mental Health Care Navigation Team. These teams provide evidence-based, strengths focused, recovery orientated assessment and treatment within a multi-disciplinary team.

The ACM program and the DHS D&A/Mental Health Care Navigation Team target those with chronic and severe drug and alcohol dependence who experience multiple and complex needs. These teams will provide support to assist in the client's stabilisation and reintegration, and where relevant, assist in managing the client's underlying cognitive impairment.

  1. Tertiary qualification as a Registered Social Worker/ Counsellor which provides eligibility for membership with a professional organisation and/or current registration with AHPRA where applicable.
  2. Demonstrated advanced skills and knowledge in triage, assessment, counselling, providing therapeutic interventions and transfer of care planning for drug and alcohol clients.
  3. Demonstrated advanced knowledge of the harm reduction approach to drug and alcohol issues and demonstrated experience in the provision of interventions with drug dependent clients.
  4. Demonstrated experience in the provision of clinical supervision to staff and or students.
  5. Demonstrated experience in managing a multidisciplinary team providing complex psychosocial interventions.
  6. High level communication skills including the ability to prepare statistical and technical reports.
  7. Demonstrated experience in leading quality improvement initiatives.
  8. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
